Leucovorin Rescue after High-Dose Methotrexate Therapy. The recommendations for leucovorin rescue are based on a methotrexate dose of 12 to 15 grams/m2 administered by intravenous infusion over 4 hours (see methotrexate package insert for full prescribing information).
Methotrexate rescue is a technique used in cancer chemotherapy. The technique involves the admininstration of leucovorin after high dose methotrexate treatment.
Leucovorin should be given 2 3 hours after glucarpidase, then every 3 6 hours at doses guided by the methotrexate concentration (pharmacokinetically guided dosing; Fig. 3) . Reduction of the plasma methotrexate concentration reduces the competition between methotrexate and leucovorin for intracellular access.
Leucovorin has no effect on other established toxicities of methotrexate such as nephrotoxicity and/or metabolite precipitation in the kidney.
